Get started on our New Client Intake here!
STEP 1: Gather all required documents needed to complete the New Client Intake Form.
- A prescription from your Primary Care Physician with your child’s diagnosis and the type of therapy requested
- A copy of the front and back of your insurance card
- A copy of the front of your driver’s license
- For ABA therapy only, a copy of your Diagnostic Paperwork from a psychologist, psychiatrist or neurologist including specific testing results and an identified diagnosis
STEP 2: Complete the New Client Form
STEP 3: Our Front Desk Specialists will reach out to you within 48 hours to confirm your submission.
STEP 4: Once our scheduling team identifies appointment time(s) that match your availability, they will reach out to you to schedule the initial evaluation.
You will receive an email confirming your appointment for the initial evaluation which will also contain a link to the Evaluation Intake Form. You should complete this form prior to your scheduled appointment.

Frequently Asked Questions
There are many factors that influence our ability to schedule new clients. It is difficult to provide an accurate estimation of when we can schedule your child’s evaluation and ongoing services. Please understand that we are consistently working to schedule clients as soon as possible. Wait times may vary from a few days to a few months. We will work to keep you informed of availability.
We take most insurance, including Medicaid, Tricare, Blue Cross Blue Shield, Aetna and many more. See a more complete list by discipline below. We also always accept private pay.
Please ensure that prior to starting services, you also verify your insurance coverage for the services prescribed by your child’s pediatrician. You can do that by calling the member service telephone number located on the back of your insurance card. All copays, coinsurance or deductible payments are due at the time of the service. If your account becomes past due it is subject to late and/or collection fees. Any services rendered which are not reimbursed by insurance are the patient’s responsibility.

As a courtesy, Great Strides will call your insurance company to obtain your coverage for the services requested and then call you to go over it before we schedule your first appointment. We occasionally are quoted incorrect benefits from insurance companies and kindly ask that you also call your insurance company to ensure we have the same information. It is your responsibility to understand your insurance coverage and benefits for services requested. If you receive a different benefit quote than we do, we may ask to do a 3-way call with you and your insurance company so that we can settle the discrepancy. Please call our Patient Account Specialist at (904)886-3228, extension 307, with any questions about your benefits.
Please arrive 15 minutes early for your first appointment, and bring with you:
- A list of questions you may have for our caring staff
- Your insurance card
- Your driver’s license
- Any previous evaluations
- Individual Education Plan (IEP)
- Your co-pay
We ask that you arrive 10 minutes early to pick up your child from therapy to go over daily progress and any other questions you may have.
If you find you have to cancel an appointment, please provide 24 hours’ notice prior to cancel your appointment. We understand that emergencies arise from time to time and ask that you call us with as much advance notice as possible. More than 3 last minute cancellations and/or no shows in a quarter will result in a $35.00 cancellation fee

In consideration of other patients and staff members, if you are going to be more than 10 minutes late for your appointment please call to ensure that your therapist can still see you. Your appointment may need to be rescheduled.

- All therapists are scheduled back to back. Late fees will be assessed for children who are not picked up on time.
Insurance companies will not reimburse for late/ cancellation fees, therefore it is the responsibility of the parents/guardians.
