There are three primary ways to make an H2 referral:
1. Download a treatment referral form and fax to H2 Health Central Scheduling at (904) 757-9679
2. Email a completed treatment referral form to referrals@h2health.com
3. Fax your referral to a clinic near you! We will take care of the rest.
